There is limited understanding of how to best leverage the promise of social determinants of health for older adults. The rise of chronic disease, an aging population, inequity in health care access and outcomes, and new technology demand a fundamentally different approach to supporting aging adults to live their most healthful life possible. This webinar will address how the National Council on Aging is driving solutions to optimize quality of life for older adults through evidence-based programs that increase healthy food knowledge and manage risks associated with malnutrition, such as falls prevention. A quality of life survey tailored for use with aging adults will be discussed. Preliminary results will be shared from a demonstration project conducted in Baltimore County including more than 10,000 seniors who self-report where they stand on a number of qualities related to physical and mental health, emotional and social support, and financial well-being.

Dorothea Vafiadis is the Director for the Center for Healthy Aging, for the National Council on Aging (NCOA), a national resource center focused on chronic disease self-management and falls prevention. She leads development efforts with public and private organizations at the national, state, and local levels with a mission to enhance quality of life for the aging population. Her portfolio includes launching NCOA’s 100 Million Healthier Lives Aging Hub with the Institute for Healthcare Improvement, a collaboration of public and private partners.
